A Specialist-Led Service with Consultant Psychiatrists and Neuropsychologist
Memory concerns rarely arrive on their own.
They can show as forgetfulness, difficulty finding words, confusion in familiar places, low mood, anxiety, or a quiet withdrawal from work and social life. Sometimes these reflect conditions that are very treatable, such as depression, thyroid imbalance, sleep problems, vitamin deficiency or medication effects. Sometimes they signal mild cognitive impairment, or an early dementia such as Alzheimer’s disease, vascular dementia, Lewy body dementia or frontotemporal dementia.
At Oxford Brain and Mind, we provide expert assessment for adults of any age who want a clear, accurate answer without a long wait, and for families seeking a rigorous second opinion or a route to early intervention. With in-house neuropsychology and consultant psychiatry, assessment, diagnosis and support sit under one roof.

Our approach considers the whole person, including cognition, mood, physical health and personal circumstances, rather than a single test score. After diagnosis, we provide ongoing review, treatment and practical support for patients and families, including advice on living well with the condition and, where appropriate, guidance on referral pathways for newer treatments.
Memory / Dementia Assessment Process
A thorough assessment follows a clear pathway. Here is each stage, from first tests to ongoing care.
Blood Tests
We will send a referral letter to your GP or a private blood test provider (such as Manor Hospital Oxford) to organise initial blood tests.
Cost paid directly to the external providerPsychiatric Assessment
The consultant psychiatrist will discuss your concerns with you and your family (or someone else who knows you well), conduct a clinical diagnostic interview and carry out a brief physical examination.
Cognitive Assessment
The consultant psychiatrist will carry out a cognitive assessment using the ACE-III or MoCA.
Brain Imaging & ECG Referral
A referral letter will be provided for a dementia-specific MRI scan and, if needed, an ECG.

Where indicated, a full neuropsychological assessment will be recommended and organised separately at Oxford Brain and Mind.
Cost paid separatelyPsychiatrist Review
Once all investigation reports are available, a review appointment is organised with the psychiatrist. Your diagnosis and treatment plans will be discussed with you.
Prescription
A prescription will be provided if appropriate.
Ongoing Care
Ongoing care with the psychiatrist and/or therapists can be organised.

Occasional forgetfulness is normal. It is worth seeking assessment when memory changes are noticeable to you or those around you, when they are getting worse, or when they begin to affect daily life, work or relationships. If in doubt, an assessment can bring reassurance as often as it brings answers.
Often, yes. Many memory changes are a normal part of ageing or are linked to stress, sleep or low mood. The purpose of a thorough assessment is to tell the difference between this and something that needs attention, so you are not left wondering.
A great many, and importantly, some are reversible. Depression, anxiety, thyroid problems, vitamin deficiency, sleep disorders and medication side-effects can all affect memory. Others, such as mild cognitive impairment and the various forms of dementia, need a clear diagnosis to guide care. A proper assessment looks at all of these.
Early, accurate diagnosis opens the most options. It allows reversible causes to be treated, gives time to plan, and is now essential for access to disease-modifying treatments for early Alzheimer’s disease, where eligibility depends on diagnosis at an early stage.
A detailed review of your history, symptoms and medication, formal cognitive assessment, and consideration of mood, sleep and physical health. Information from someone who knows you well is needed as part of the assessment. We will advise on appropriate investigations, including brain imaging where indicated. You will receive a clear written report and a personalised plan.
A diagnosis is the beginning of care, not the end of it. We are able to provide ongoing reviews and medical treatment as appropriate. We will refer for practical support for patients and families. We will also ensure we work with other professionals involved in your care.
