Repetitive Transcranial Magnetic Stimulation (rTMS)
Oxford Brain and Mind is a specialised service for Repetitive Transcranial Magnetic Stimulation (rTMS). We have the expertise of Dr Sanjay Kumar (Senior Lecturer at Oxford Brookes University and Director of Oxford Brain and Mind) who has an extensive scientific background in rTMS research.
We have clinical expertise from our Consultant Psychiatrist Dr Tina Malhotra, who will do an initial assessment and decide whether rTMS is an appropriate treatment option for you.
We also take referrals from other private Psychiatrists if they recommend rTMS for you. In that case we will still do a brief psychiatric consultation to assess for a suitable rTMS protocol, safety screening, medical history and check for any contraindications.
We will offer a bespoke rTMS protocol for you depending upon your schedule and clinical indication.
What is rTMS?
rTMS is a non-invasive treatment for depression, anxiety disorders and various other psychiatric conditions. It can be used in conjunction with medication and psychotherapy or as a standalone option.
Based on randomised controlled trials, rTMS has proven to be an effective treatment. The National Institute of Clinical Excellence (NICE, UK) has included rTMS as a proven treatment for depression in its guidance since 2015. We use MagVenture rTMS system who has received CE approval for non-invasive treatment of addiction, OCD and comorbid anxiety.
How does rTMS work? Recent advances in understanding brain based mechanisms in psychiatric conditions has led to significant advancement in treatments directly stimulating the brain. rTMS is an innovative treatment that uses repetitive pulses of magnetic field to target abnormal brain circuits implicated in psychiatric conditions. Over the course of treatment with rTMS, there is a neuroplastic effect on the brain, which means that it restores and improves the brain’s ability to repair and build new connections between the nerve cells, thereby improving clinical symptoms.
What we offer?
We offer an initial comprehensive assessment by the Consultant Psychiatrist to make a decision regarding your suitability and clinical need for rTMS. They will also discuss an appropriate rTMS protocol with you. The clinical conditions we treat with rTMS are Depression , Anxiety disorders and OCD (hyper link to these conditions)
A typical protocol for the full course of rTMS treatment involves 20 to 30 sessions over 4 to 6 weeks. There are other protocols that can be shorter but with more frequent treatments in a day. Depending upon the clinical need and your schedule we can offer a bespoke rTMS protocol for you.
At Oxford Brain and Mind, we also offer an additional scientifically based, short computer based cognitive task before every rTMS session. The use of cognitive tasks is designed to pre-activate the brain circuits associated with the psychiatric disorder so that the effect of rTMS is enhanced. This task is on top of the NICE approved rTMS protocol for treatment of depression. Preliminary scientific understanding points that it is a useful adjunct to rTMS treatment.
Is rTMS safe?
rTMS is a non-invasive treatment which has proven to be safe in all clinical trials. The minimal side effects reported with rTMS are short lasting mild headache and mild fatigue.
It does not have side effects associated with medication and is sometimes preferred over medication if the response to medication is poor or you can’t tolerate side effects of the medication.
There are certain conditions where the application of rTMS should be avoided. Your clinician will discuss about the suitability of rTMS with you.
